Important Information Regarding Registration

It is important to complete all steps in the registration process and to have all paperwork completed to register. When online registration is complete, bring a copy of the completed online registration and provide the following components to the district office at 921 Susana Street.

 

Please Bring Originals

 

Ø  Proof of Birth Date – (Birth certificate, baptismal certificate, passport) Office staff must verify your child’s birth date. This information will be returned to you. Your child will not be admitted into without proof of birth date.

Ø  Declaration of Residency - (3 proofs) such as: Property tax statement (current year), Utility service contract, statement or payment receipts (current month), Rental property contract, lease or payment receipt (current month), Pay stub (current month), Correspondence from a government agency, Declaration of residency executed by the parent or legal guardian of pupil. Your child will not be admitted without proof of residence.

Ø  Immunizations – Immunizations Administration Record

 

All registration is done at the district office located at 921 Susana Street.
